We are looking to employ an experienced Practice Manager who is passionate about developing, mentoring and guiding a newly created allied health team. The position is full time and located in Sydney.

The Role
The Practice Manager will be responsible for providing high level practice management of complex care participants and providing support and direction, ensuring the practice remains financially sustainable whilst delivering excellence of customer service.
About You
Our ideal candidate will have worked as a practice manager previously, within the disability/aged care/health system. The role will see you providing leadership amongst an established team whilst managing the day to day operations of the services across the region.
The successful candidate will hold a tertiary qualification in Nursing/Mental Health/Disability/Community Services or relevant experience as a practice manager in a clinical setting. Knowledge of NDIS processes would be an advantage.

Essential
- Tertiary Degree in Social Work, Psychology, Community Services, or a related field, or working towards one.
- Minimum 2-5 years of industry experience in a similar role will be considered.
- Background in Disability and/or Aged Care is essential.
- Experience in mental health, nursing, or healthcare within aged/disability or a related field.
- Availability and flexibility to work over a 7-day roster.
- Proficient in Windows-based software systems

Position: Key Support Worker
Job Description:
Key Support worker Responsibilities and Daily Duties need to perform.
1. Concerns and Complaints:
- Report, record, and address any concerns in the participant’s interest.
- Support participants or nominees in making complaints or changing support services.
2. Participant Well-being:
- Ensure the participant’s needs are met and NDIS goals are on track.
- Advocate for the participant’s choices, control, and pursuit of personal interests.
3. Upholding Rights and Choices:
- Promote and protect individual rights, including freedom of expression and decision-making.
- Support participants in everyday decisions and more complex co-design of services.
4. Communication and Liaison:
- Liaise with key stakeholders to relay support requests and take appropriate actions.
- Be the go-to person for addressing and resolving concerns raised by various parties.
5. Support Plan Management:
- Brief other support workers on alterations to the support plan.
- Ensure everyone is involved and understands the purpose of the Key Support Worker role.
6. Transparent Communication:
- Communicate new participant requests or support changes with other workers.
- Discuss duty delegations, ensuring everyone understands details and availability.
7. Confidentiality and Consent:
- Keep participant information confidential and obtain consent for sharing details.
- Ensure participants understand the purpose and reason behind sharing their information.
8. Monitoring Professionalism:
- Check for self-regulation and maintain professional boundaries among support workers.
- Address any issues that may negatively impact the participant or the team.
9. Collaboration and Progress Reports:
- Collaborate with support workers to obtain progress reports for plan reviews.
- Ensure information is collected professionally and in a simple, comprehensible manner.
Qualifications
Essential:
- Minimum Certificate IV in Community Services/Disability care/Age care or related discipline
Desirable:
- Relevant tertiary qualification in Social Worker, Psychology or related discipline
Experience
Essential:
- Previous experience in a similar role in out of home care
